If your situation is complicated, or you are looking after someone with complex needs, our support planning service is here to make things easier.
Who can benefit from support planning?
Our Personalised Support Planning service can help you if:
- You want to arrange care for a family member or friend – but you don’t have the time or the knowledge of what’s available
- You’ve recently had a diagnosis of a long-term illness – and you’d like to organise the support you’ll need in the future
- You want to make a change to your life – but you struggle to put plans into action, and you need someone to help you set goals you can achieve
What does the service involve?
Stage 1: Assessment
We’ll start with a detailed assessment of your situation or the situation of the person you care for. This is personalised to each person and is led by your priorities concerns and priorities. By the end, we’ll have a comprehensive picture of your life.
Stage 2: Planning
This stage involves turning your needs into a concrete action plan with clear, achievable goals. We will review progress against these goals in the third stage. If you opt for support brokerage, we’ll also include what actions the support broker will take in this plan.
Optional: Support brokerage
If you don’t want to arrange support services yourself, our support broker can do this on behalf. They will estimate how long it should take before support is fully in place and will keep you up to date with progress. Support brokerage has an hourly cost (see below).
Stage 3: Review
We’ll meet again after a few weeks to see how things how you and the support broker have progressed against the goals you set out in the planning stage. This is also an opportunity to reflect on what support is working and whether you might want to make a change.
How much does the service cost?
The standard cost of our Support Planning service is £75 and covers:
- 1 hour in-depth assessment of your needs or the needs of the person you care for
- 2 hours of support planning
- A follow-up session.
Support Brokerage is charged at an hourly rate of £25. Your support broker will give you an estimate of the overall cost before you agree to the service and will keep you informed of anything that could delay the process.
